Below you will find Health Certificate forms for both International and Interstate travel. If you are traveling with your pet soon, it’s important that you follow all appropriate guidelines from your travel destination and allow enough time for us to provide you with your health certificate. Due to 3rd party lab protocols, which are outside of Nipomo Dog & Cat Hospital’s control, they currently do not offer rush/stat services and it may take up to 8 weeks for results.
Please Note: We are only doing health certificates for current and active patients upon the veterinarian’s approval. It is required that you email us completed copies of the following applicable forms to info@nipomodogandcat.com prior to your scheduled appointment.
